(i) Selected candidates are advised to deposit their fee from 27th December to 29th December 2022 during office hours.
(ii) Selected candidates have to submit following documents in original.
- Matriculation Certificate (SSC) or O-level Equivalence Certificate by IBCC.
- Matriculation marks certificate or 'O' level Transcript Certificate by IBCC.
- HSC Pre-Medical certificate (Provisional) or A-Level equivalence certificate by IBCC.
- HSSC Pre-Medical marks Certificate / A Level Transcript Certificate by IBCC.
- Computerized National Identity Card from / 'B' Form of candidate (Photocopy).
- Domicile Certificate of candidate.
- PRC Certificate of candidate.
- Domicile of Father, if father is not alive then Mother (along-with Death Certificate issued by NADRA)
- Computerized National Identity Card of Father (in case of death of father or divorce) then Mother (Photocopy)
- Affidavit / Undertaking (Rs.100)
